Insurance Cards

A copy of your insurance card is needed each time you visit. The card contains important information which is required to complete your insurance claim. If we do not get all of your information, it may be necessary to bill you for medical services rendered by our clinicians.

Co-Pay

Many insurance companies require that you pay a co-pay for each office visit. This applies to office visits with a physician specialists, also. Be prepared to pay your co-pay prior to seeing the physician. We are required by your insurance company to collect the co-pay.

Referrals

Some insurance companies require that you have a referral from your family physician before our physicians can see you. This referral may be a piece of paper with instructions to our doctors. The referral may also have a number that we are required to record on the claim form that we submit to your insurance company. The referral can be faxed directly to the office location you are visiting, but you can help us by confirming that your family doctor completed the paperwork in advance of your appointment.

Surgery

Should surgery be required, our office will obtain necessary pre-certification. Your insurance company may require a deductible or co-pay. However, pre-certification of a procedure is not a guarantee of payment by your insurance company. Payment is determined at the time the claim is processed at the insurancy company. If you get a bill related to any services you received payment is expected upon reciept of the bill.
