New Patient Information
To be sure we fully understand your medical needs, it is important we have information about your current medical condition. Please arrive 20 minutes prior to your appointment so you can be seen at your appointed time. Please provide all the information requested in the following areas.
Medications
Please bring a list of all your medications along with the dose and schedule to your appointment. These include prescription, over the counter, and herbal medications.
Past Medical History
Ask your primary care doctor to provide a summary of your medical condition to bring to your appointment, or provide a list yourself.
If you have had a CT scan, MRI, Sleep Study or other tests done as a result of your current medical symptoms, bring those written test results and films with you. Hand carry the films to the appointment.
Insurance Cards
We copy your insurance card every visit. The card contains valuable information, which is required by your insurance company, to file the claim. If we do not to get all your insurance information, it will be necessary to bill you for the services.
Co-Pay
Many insurance companies require that you pay a co-pay for each office visit. That applies to office visits with a specialist also. Be prepared to pay your co-pay prior to seeing the physician. We are required by your insurance company to collect the co-pay. A $15 processing fee will be assessed if co-pay is not paid at the time of the visit.
Referral
Many insurance companies have relaxed their referral criteria and only require a written note from your personal doctor to see a specialist. Some still require a written referral with a specific number. Please bring a copy of the doctor's request to be seen by a specialist.
Surgery
Should surgery be required, our office will obtain the necessary pre-certification. Your insurance company may require a deductible or co-pay.
